1. Call your insurance company to confirm your benefits
2. Go to a healthcare provider that accepts your insurance
I am uninsured/ My insurance does not cover the procedures
1. Call 1-866-442-CANCER (2262) to verify eligibility and where you can go
2. Go to your nearest CSP

- Mammograms every 1-2 years for women 40+
- Cervical cancer screening every 3 years
- DNA tests for HPV
- HPV vaccination
All plans after August 1, 2012 are eligible!

- Medicaid/Medicare

- No insurance or insurance does not cover screening exams
- Household income at or below 250% of federal poverty level
- NY Resident
- Breast Cancer Screening: 40 – 64 years of age
- Cervical Cancer Screening: 21 – 64 years of age
Services:
- Clinical breast exams
- Mammograms
- Pap tests
- Human papilloma virus (HPV) tests
- Pelvic exams
- Diagnostic testing if results are abnormal
- Referrals for treatment
